Description:  The Executive Director, Care Management oversees and directs the operational effectiveness of care coordination, and clinical integration activities.  This job encompasses leadership of care coordination, utilization review, social work services, and discharge planning.

  1. Directs the recruiting, hiring, supervision and evaluation for department staff while providing discipline, counseling and coaching.
  2. Coordinates duties with appropriate staff to meet operation program needs and ensures compliance with state and federal requirements, Medicare guidelines and accreditation standards.  Develops and implements policy and procedures.
  3. Develops and implements a care coordination model that provides case management services and support with the intent to improve the efficiency and effectiveness of the delivery of care coordination.
  4. Plans, organizes and implements processes to provide utilization reviews, case management, care coordination and discharge planning services. 
  5. Develops quality, financial and utilization indicators based on accepted standards.  Reports indicators to senior management, utilization management and medical executive committee and to the board of managers at least quarterly. (Recommend monthly report to Sr. Management and Utilization Management).
  6. Oversees the development and implementation of an effective Care and Outcomes Management Plan as well as develops and implements a comprehensive outcomes management plan that addresses clinical, operational and financial outcomes.  
  7. Serves as a resource to the Executive, Physician and operational department leaders in developing strategies and indicators to improve care.
  8. Performs an annual evaluation of the effectiveness of monitoring and improvement efforts and makes recommendations for modifying the plan or emphasis of improvement efforts for the next twelve months.
  9. Directs and implements alignment of initiatives and processes which will improve cohort management and improve clinical and financial outcomes.
  10. Defines and oversees implementation of a population stratification methodology that support identification of subpopulations that require specific services at specified intervals
  11. Assumes a leadership role in quality improvement activities and Waiver initiatives.
  12. Prepares, reviews, and manages the department budget to meet or exceed budgetary expectations.
  13. Performs other related job duties as assigned.
